Geriatric Care Management
With our complete range of Geriatric Care Management services, Senior Solutions will make sure that you’re providing your loved one with the care he or she needs most. With your participation every step of the way, Senior Solutions can:
- Consult with you and your family to identify your particular needs and eligibility for assistance
- Conduct thorough assessments of your situation and develop an appropriate care plan
- Screen, arrange, and monitor in-home help or other recommended services
- Provide assistance in evaluation of nursing homes or other living arrangements
- Monitor frail individuals in any living environment—home, hospital, nursing home, or assisted living facility
- Assist with moving an individual to or from a retirement community or nursing home
- Review applicable financial, legal, and/or medical issues with physicians and attorneys
- Offer referrals to geriatric specialists
- Assess homes and apartments for safety and comfort
- Act as a liaison for out-of-town relatives
- Assume responsibility for healthcare power of attorney
- Provide visits and phone contact for individuals who live alone
- Recommend the help of Caring Companions or other appropriate home health agency when a home health aid is needed
Ongoing Care Management
To help maintain maximum independence, Senior Solutions can visit your family member regularly, accompany them to doctor’s appointments, monitor medications, and advocate on their behalf with physicians or assisted living providers. Senior Solutions will keep you informed during every step of the ongoing care process so you can help us make the best decisions regarding the care of your loved one.
In providing the above services, Senior Solutions remains accessible at all times and is available after hours and on weekends in the case of emergencies. We are efficient and flexible, with a personal touch.
